Apendicitis Por Sonografia

El diagnostico sonográfico de Apendicitis es difícil. También clínicamente es un enfermedad elusiva, cambiante, con muchas de sus manifestaciones clínicas que varían ampliamente, incluso dependiendo de la edad del paciente y de otras variables. Lo clásico de dolor en epigastrio ( estómago ) que luego se localiza en fosa iliaca derecha ( parte derecha, baja, del vientre) , lo vemos pocas veces, es cuando llamamos una apendicitis " de libro " Un factor a considerar siempre es si el paciente ha tomado algún analgésico, eso altera el curso natural de la enfermedad y provoca que los síntomas clásicos " se oculten ", por lo menos por algún tiempo y provoca , en ocasiones, graves equivocaciones. Recuerdo de mi periodo Norteamericano, en el Hospital de Danbury, Connecticut , Departamento de Patología, el dato de que un porcentaje importante de las Apéndices que se envían a Patología, no son Apendicitis en realidad ( más de la mitad ). Partiendo de esos hechos, cada vez que me encomiendan aclarar si una Apendicitis es real o no, siento cierto grado de aprensión. Hace unos días, me llamaron de Emergencia para aclarar si se trataba de una Apendicitis o no. Se trataba de un paciente masculino, de 57 años de edad, que llevaba varios días con vagos malestares en todo el vientre, con algún grado de focalización en la zona apendicular. El paciente estaba relacionado con médicos y lucía tener Educación Superior y se auto-medicaba con Analgésicos y medicamentos para malestares gástricos e Intestinales, pues el asumía que se trataba de que " algo le cayó mal "
Después del interrogatorio clínico de rigor y de la palpación abdominal y de ver los análisis realizados hasta ese momento, que son etapas obligadas en mi rutina antes de usar la Sonografía, procedí al examen propiamente dicho. En éste se apreciaba lo siguiente:

The sonographic diagnosis of Appendicitis is difficult. It is also clinically an elusive, changeable disease, with many of its clinical manifestations that vary widely, even depending on the age of the patient and other variables. The classic type of pain in the epigastrium (stomach) that is later located in the right iliac fossa (right, the lower part of the belly), we see it rarely, it is when we call it a "book" appendicitis. has taken any painkiller, this alters the natural course of the disease and causes the classic symptoms to "hide", at least for some time, and sometimes causes serious mistakes. I remember from my North American period, at Danbury Hospital, Connecticut, Department of Pathology, the fact that a significant percentage of the Appendices that are sent to Pathology is not actually Appendicitis (more than half). Based on these facts, every time I am asked to clarify whether Appendicitis is real or not, I feel a certain degree of apprehension. A few days ago, I was called to the Emergency Department to clarify whether it was appendicitis or not. It was a 57-year-old male patient who had had vague discomforts throughout the belly for several days, with some degree of focus on the appendicular area. The patient was related to doctors and seemed to have Higher Education and self-medicated with Analgesics and medications for gastric and intestinal discomfort because he assumed that it was that "something went wrong"
After the rigorous clinical questioning and abdominal palpation and seeing the analyzes carried out up to that moment, which is mandatory stages in my routine before using Sonography, I proceeded to the examination itself. In this one, the following was appreciated:















En estas imágenes de la zona apendicular pudimos diagnosticar con relativa seguridad, que se trataba de lo que llamamos " Plastron Apendicular " que se da en Apendicitis de relativa larga duración y donde el proceso inflamatorio focal trata de ser contenido por la membrana peritoneal que "arropa" toda el área. En estas dos fotos se pueden aprecian la masa irregular de todo el complejo, el derrame liquido inflamatorio que rodea toda la masa y la presencia de un "Fecalito" que lo señalamos como calcificación y que se visualiza cuando hay obstrucción del área apendicular.
Con estos datos se procedió a la intervención quirúrgica durante la cual se encontró el antedicho " Plastrón  " el cual sufrió una ruptura en el momento de ser extraído, si se hubiera tardado sólo unas horas, el resultado habría sido una Peritonitis, con pronóstico bastante peor.
Al día siguiente pasé por la habitación del paciente y me lo encontré sin dolor,sonriente, feliz, pero lo que el no sabe es que más feliz estaba yo, por haber puesto " mi granito de arena" , para resolver un caso complejo y que puso en riesgo una vida.

With these data we proceeded to the surgical intervention during which the aforementioned "Plastron" was found, which suffered a rupture at the time of being extracted, if it had taken only a few hours, the result would have been Peritonitis, with a much worse prognosis.
The next day I went to the patient's room and I found him without pain, smiling, happy, but what he does not know is that I was happier, for having done "my grain of sand", to solve a complex case and that put a life at risk.
