Metastasis Hepaticas

Femenina 49 años , operada de mama izquierda ( mastectomia total ) en el año 2005, viene a revisión sonográfica de abdomen. Previo a este examen , en otro centro, le habían dicho que tenia " hígado graso " o sea esteatosis hepática, que, dicho sea de paso, es un diagnostico sonográfico del cual se está abusando, etiquetando pacientes en esta categoría, sin cumplir los parámetros exigible para ello. El hígado mostró múltiples imagenes nodulares, irregulares en su forma, la mayoría de ellas de pequeño tamaño, se distribuían por igual en ambos lóbulos hepáticos.El nódulo de mayor tamaño se localiza en lóbulo derecho muestra un aspecto sonográfico de " ojo de buey " es hiper-ecogenico, con pequeña área central hipo-ecogenica y rodeado de halo hipo-ecogenico. Al Doppler de poder se manifiesta un aumento discreto del flujo vascular al nódulo y a la zona peri-nodular.( ver imagenes )

En la primera imagen se aprecian varios nódulos hipo-ecogenicos localizados en lóbulo izquierdo y en la frontera de este con el lóbulo derecho. En la segunda imagen se aprecia el gran nódulo del lóbulo derecho con dos pequeñas calcificaciones en sus bordes. En la tercera imagen, apreciamos las dimensiones del nódulo de 4.3 X 3.9 cm y en la cuarta y última imagen el Doppler de poder mostrando el flujo en y hacia el nódulo hepático. Para finalizar la famosa pregunta del paciente que hace que se me atraganten las palabras. Doctor el hígado graso es algo malo ?? Como comprenderan, es difícil contestar honestamente tal pregunta

Liver Metastases
