Tiroiditis Hashimoto
Femenina. 21 años de edad. Antecedentes familiares ( Abuela y tia materna) de Hipotiroisimo. Comienza hace un mes con perdida de peso importante ( a pesar de comer bastante ), nerviosismo, tremor en manos, sudoraciones frias, exoftalmos, insomnio, caída pelos, rotura fácil de uñas, etc.
Al examen físico, se aprecia exoftalmos no muy pronunciado, la paciente luce muy nerviosa y tensa, presenta masa de tacto gomoso en cara anterior de cuello, que se corresponde con la silueta agrandada de la tiroides. La sonografia mostró marcado aumento del volumen tiroideo, con hipoecogenicidad generalizada y aspecto micro-nodular que se distribuye de manera homogenea por ambos lóbulos y por el istmo tiroideo.
A la exploración Doppler se aprecia un significativo aumento del flujo vascular a todo el órgano, siendo mas marcado en lóbulo derecho, tal como se aprecia en estas dos imagenes

Hashimoto's Thyroiditis
Female 21 years old. Family history ( grandmother and maternal aunt) of Hipotiroisimo. It starts a month ago with major weight loss (despite eating enough), nervousness, tremor in hands, cold sweats, exophthalmos, insomnia, hair fall, easy to break nails, etc.
On a physical examination, there is not a very pronounced exophthalmos, the patient looks very nervous and tense, presents a touch rubbery mass in front of the neck, which corresponds with the shape of the enlarged thyroid. The sonography showed a marked increase in thyroid volume, with hypoechoic and widespread micro-nodular appearance, which is distributed uniformly on both lobes and isthmus of the thyroid.
