Paciente femenina 87 años de edad. Conteo de plaquetas el 13 Mayo 2009: 896 mil ,
al día siguiente ( 14-Mayo 2009 ) 868 mil ( normal entre 150-45o mil ).

Clinica al ingresar: Mareos y Afasia Motora. La radiografia de torax mostró Cardiomegalia severa con dilatacion y calcificacion ligera de boton aortico y discreto sindrome intesticial pulmonar. La tomografia axial computerizada ( TAC ) de craneo : evento vascular antiguo en lobulo temporal izquierdo con cambios vasculares difusos y atrofia cortical.
El ecocardiograma insuficiencia tricuspide , signos de cardiopatia hipertensiva y dilatacion de cavidades derechas con insuficiencia mitral moderada y funcion sistolica normal.
En la sonografia Hígado aumentado de tamaño con heterogeneidad focal por la presencia de múltiples imagenes quisticas, tabicadas, con micro-calcificaciones en sus paredes.La lesión quistica de mayor tamaño se localiza en lóbulo derecho, mide aprox: 4.9 X 3.9 X 4.1 cm, con volumen de 36.69 c.c, el de menor tamaño mide aprox: 2.6 X 2.2 X 2.0 cm con volumen de 5.49 c.c.
En lóbulo derecho se visualiza masa solida, ecogena, localizada en la porción inferior del lóbulo derecho, esta rodeada de halo an-ecogeno, mide aprox: 6.7 X 6.4 X 6.2 cms.
El departamento de Hematologia opina que el cuadro puede deberse a: Cuadro Para-Neoplasico o Sindrome mieloproliferativo tipo trombocitopenia esencial

Enter the clinic: Dizziness and Motor Aphasia.
Chest radiography showed severe cardiomegaly with a dilated aortic calcification lightweight and discrete button interstitial pulmonary syndrome.
Computed axial tomography (CT) brain: a vascular event in the old left temporal lobe with vascular changes and diffuse cortical atrophy.
The echocardiogram tricuspid insufficiency and signs of hypertensive cardiopathy dilated right cavities with moderate mitral insufficiency and normal systolic function.
Liver sonography in the enlarged focal heterogeneity with the presence of multiple cystic images, enclosed, with micro-calcifications in their walls. The larger cystic lesion located in the right lobe, measures approx: 4.9 X 3.9 X 4.1 cm with a volume of 36.69 ccs, the smallest measures approx: 2.6 X 2.2 X 2.0 cm with a volume of 5.49 cc
The right lobe is shown in a solid mass, echogenic, located in the lower right lobe, is surrounded by an an-echogenic halo, measures approx: 6.7 X 6.4 X 6.2 cms.
Hematologia department believes that the painting may be due to: -Picture-neoplastic -Myeloproliferative syndrome type essential thrombocytopenia
