Cuando un Paciente Oculta algo...

En ocasiones nos llegan pacientes que han sido visto y/ o diagnosticados en otros centros y mantienen un hermetismo que no se resuelve hasta que le explicamos " nuestro diagnostico " , entonces y solo entonces, nos cuentan del examen anterior, coincida o no con el nuestro. Muchas veces lo hacen para " probar " nuestra capacidad.
Esto viene a colación porque examinamos a una paciente del Tiroides y para nuestra inicial sorpresa, encontramos que el esófago tenia un recorrido anómalo, pues aparecía a la derecha o no a la izquierda como es habitual. En principio pensamos que habíamos invertido la imagen o el transductor, pero, tras verificar que estábamos correcto, cambiamos de transductor y examinamos el hipocondrio derecho con transductor sectorial de 3.5 Mhz, lo cual confirmo nuestra sospecha, se trataba de una paciente con situs inversus total, o sea, todos sus órganos estaban invertidos: Corazón a la derecha, Hígado a la izquierda, Bazo a la derecha etc ( ver caso nuestro similar en situs-inversus ).En las imagenes que siguen se puede apreciar el lóbulo derecho del tiroides ( o es el izquierdo ? ) y la arteria Carótida y la Vena Yugular derecha ? ? de la paciente y la presencia de la imagen tubular característica del esófago que colocado a la derecha fue visualizado y durante el examen le pedimos a la paciente que deglutiera saliva para comprobar y confirmar sin lugar a dudas de que se trataba del esófago, además de que del lado izquierdo no había imagen similar.
Ya terminado el examen sonográfico la paciente nos confirmo que desde hacia años sabia de su condición ya que fue descubierta por accidente al realizarle una radiografia de tórax como examen previo por una histerectomia que se le iba a realizar. Posteriormente sufrió Apendicectomia que se realizo al través de la cicatriz central de la Histerectomia y que según la paciente fue muy laboriosa ya que no encontraban el Apéndice en el lado izquierdo, a pesar de que los cirujanos conocían su condición de situs inversus. La paciente tiene 58 años y segun confiesa una salud muy buena.

At first, we thought we had invested the image or the transducer, but, after checking that we were correct, we changed the transducer and examined the right upper quadrant with 3.5 MHz sector transducer, which confirms our suspicions, it was a patient with a total situs inversus, ie, all organs were reversed: Heart on the right, left Liver, Spleen right, etc (see a similar case in our previous case-situs-inversus). In the pictures below you can see the right lobe Thyroid (or is it left?) and carotid artery and jugular vein right? ? of the patient and the presence of the image feature tubular esophagus placed on the right, was displayed during the test and ask the patient to swallow saliva to check and confirm without a doubt that it was the esophagus besides that the left side there was a similar picture.

Sonographic examination already completed the patient will confirm that for years and knew of his condition that was discovered by accident by an X-ray examination of the chest and a prior hysterectomy to it going to take place. Subsequently underwent the appendectomy that was performed through the central scar of hysterectomy as the patient was very laborious and could not find the appendix on the left side, although the surgeons were aware of their condition with situs inversus The patient is 58 years old and according to confesses very good health.
