Tras una Cesarea.....

Paciente femenina de 29 años de edad ingresa tras cesárea realizada en otro centro hace 7 días. Presenta fiebre desde su cirugía y sangrado trans-vaginal. Se planea curetage pero el ginecólogo desea saber con exactitud cual es el problema. El examen eco-sonográfico abdominal no encontró ningún foco de absceso sub-frenico o derrame pleurales. El examen pelvico trans-vaginal mostró útero aumentado de tamaño ( 12.0 X 8.9 X 8.0 cm ) con cavidad endometrial real ocupada por material liquido con abundantes grumos con apariencia sonográfica de ser de origen hematico. A 1.2 cms del os interno, en cara anterior del segmento inferior uterino y prácticamente a nivel de la cicatriz quirúrgica de la cesárea, se visualiza nódulo ecogeno, homogéneo, de unos 1,2 X 1,7 cm, presumiblemente producido por restos placentarios. Se planifica legrado terapéutico..Se realiza el procedimiento y se hace cubierta antibiótica, al día siguiente la paciente refiere desaparición del sangrado y la fiebre cedió junto con descenso significativo de la leucocitosis que la paciente presentaba. El examen histopatologico del material del legrado fue Miometritis


After Caesarea .....

A female patient aged 29 was admitted after cesarean section at another hospital for 7 days. Fever since her surgery and transvaginal bleeding. Curettage is planned but the OB wants to know exactly what the problem. The eco-sonographic abdominal examination found no source of sub-phrenic abscess or pleural effusion. The transvaginal pelvic examination showed an enlarged uterus ( 12.0 X 8.9 X 8.0 cm ) with a real endometrial cavity occupied by liquid material with abundant clumps with the sonographic appearance of being home bloody. A 1.2 cm of the internal os in front of the lower uterine segment and almost at the level of the surgical scar of cesarean section were visualized echogenic nodule, homogeneous, about 1.2 X 1.7 cm, presumably caused by placental remains. If the procedure and antibiotic cover are made, the next day the patient reported the disappearance of bleeding and fever gave along with a significant decrease in the patient had leukocytosis. The histopathological examination of the curettage material was myometritis.
