Pielonefritis Aguda & Epididimitis



Paciente de 65 años de edad, trasplantado renal desde Marzo 2012, presenta cuadro febril intenso por infección urinaria. Sufrió de Insuficiencia Renal Crónica ( IRC) a causa de la Enfermedad de Berger o Nefropatia por inmunoglobulina A.
EL examen sonografico abdominal mostró ambos riñones originales con marcada atrofia y un riñón trasplantado con focos hipo-ecogenicos diseminados por el cortex renal con una silueta renal alterada, compatible con el diagnostico de Pielonefritis Aguda. Durante el examen sonografico se apersono la medico tratante y me pidió ampliar el examen hacia los testiculos para tratar de encontrar la causa o el foco de la infección ya que el paciente se queja de dolor testicular derecho, el examen mostró un hidrocele derecho, leve, con tabiques . El epididimo derecho se apreció engrosado sobre todo el cola donde alcanza un aspecto de mazo, con un grosor de aprox 13,2 mm.
Este hallazgo muestra lo importante de la comunicación medico tratante-medico sonografista que redunda en beneficio para el paciente.

Pyelonephritis Acute & Epididymitis


65-year-old kidney transplant since March 2012, has intense febrile UTI. He suffered from chronic renal failure (CRF) due to disease or nephropathy Berger immunoglobulin A.
The abdominal sonographic examination showed both native kidneys with marked atrophy and a transplanted kidney hypo-echogenic foci scattered throughout the renal cortex with an impaired renal silhouette, compatible with the diagnosis of acute pyelonephritis. During the sonographic examination, she appeared before the treating physician and asked me to extend the examination to the testicles to try to find the cause or the source of the infection and the patient complains of pain right testicle examination showed a right hydrocele, slight, with partitions. The thickened appreciation right epididymis especially where the queue reaches a harness aspect, with a thickness of about 13.2 mm.
This finding shows the importance of treating physician communication, medical sonographer ranging benefits for the patient.

El cuerpo lúteo es una de las señales indirectas del embarazo durante el primer trimestre. Nos permite decirle a la paciente cual de los ovarios fue el que produjo el óvulo que fue fecundado. Mostramos en esta entrega dos imágenes del embarazo de 8 semanas ( 2 meses ) de una paciente de 21 años de edad . Calcificaciones Hepaticas Heterotopicas

Las calcificaciones heterotópicas del hígado pueden presentarse visualmente de forma diferente en las exploraciones sonograficas. Mostramos tres ejemplos de esto: En la primera imagen la forma más frecuente de depósito de calcio en forma heterotópica en el tejido hepático, con una forma granular, muestra sombra ecogénica posterior, lo cual demuestra su carácter de calcificacion. Se trata de una niña de dos años y diez meses de edad que publicamos en Diciembre del 2008. El depósito se localiza en la cisura inter-lobar del lóbulo Hepático derecho.
