Quiste en el Cuello

 

Paciente femenina de 59 años de edad con nódulo palpable, duro, no doloroso espontáneamente o a la palpación , mobilizable  , localizado en cara lateral izquierda del cuello, por detrás del borde posterior del musculo esterno-cleido-mastoideo. Dice tener dos años desde su aparición y que en ocasiones ha tenido mayor tamaño ( como un huevo de gallina dice ). Ha tomado antibióticos y anti-inflamatorios en la falsa creencia de que se trataba de un adenomegalia cervical. Para descartar eso y tratar de evidenciar la presencia de otras adenomegalias cervicales fue remitida para estudio sonografico.
El examen del área muestra la presencia de tres imágenes quisticas coalescentes, con algunos grumos finos en su interior, son eco-doppler negativos y se localizan por fuera de los vasos del cuello ( Carótida y Yugular ) y en contacto con la fascia posterior del musculo esterno-cleido mastoideo , en el triangulo posterior izquierdo del cuello.
El diagnostico diferencial fundamentalmente es entre el quiste Dermoide que esta formado por  elementos ectodérmicos o restos embrionarios, compuestos por estructuras rudimentarias o bien desarrolladas, como glándulas sebáceas, tejido conectivo y folículos pilosos. Comúnmente crecen lateralmente por acumulo de material sebáceo, son subcutáneos, adheridos a planos profundos, de consistencia dura, también pueden encontrarse en la línea media. La resección quirúrgica es el tratamiento de elección, ya que este tipo de lesiones se infectan recurrentemente.


Quiste Branquial.... que son remanentes o vestigios del aparato branquial, del cual se derivan las diversas estructuras del cuello; existen cinco arcos branquiales y cuatro hendiduras, de éstos el segundo arco branquial es el área mas común del que se originan los quistes. La posición del quiste depende de la relación con cada arco branquial. Generalmente, se presentan como una protuberancia a nivel del borde anterior del esternocleidomastoideo
 pueden o no tener trayecto fistuloso. Al igual que los quistes tiroglosos, éstos se pueden manifestar a cualquier edad, pero la mayoría de veces se diagnostican en las primeras dos décadas de la vida; el diagnóstico es mas evidente cuando existe fístula asociada, incluso el primer síntoma puede ser la sobreinfección de éste conducto. El tratamiento es quirúrgico, se requiere solucionar la infección antes de la cirugía.

Otra posibilidad para el diagnostico diferencial es el tumor del cuerpo carotideo o del glomus carotideo , pero en este caso se descarta por no existir datos de vasos en el interior de la lesión cuando la examinamos con el Doppler.


Cyst in Neck


Female patient, 59 years of age with a palpable lump, hard, painless spontaneously or tenderness, mobilizable, located in the left lateral neck, behind the back edge of the sternomastoid-cleido muscle. She says has two years since its inception and has sometimes larger (as a chicken egg says). She has been on antibiotics and anti-inflammatory in the false belief that it was cervical lymphadenopathy. To dismiss that and try to detect the presence of other cervical lymphadenopathy was referred for a sonographic study.
The sonographic examination of the area shows the presence of three coalescing cystic images, with some fine lumps inside, are negative Doppler and are located outside of the vessels of the neck (carotid and jugular) and contact the rear fascia sternomastoid cleido muscle left, in the left posterior triangle of the neck.

The differential diagnosis between the - Dermoid cyst: ectodermal elements or traces are embryonic or rudimentary structures composed of well-developed, and sebaceous glands, connective tissue, and hair follicles. Commonly grow laterally by the accumulation of sebaceous material, are subcutaneous, attached to a deep, hard consistency, are also found in the midline. Surgical resection is the treatment of choice, as these types of injuries are infected repeatedly.


Or Branchial cyst ......are remnants or vestiges of the gill apparatus, from which are derived the various structures of the neck, as previously mentioned, there are five-gill arches and four slots, of which the second branchial arch is the most common area of the originating cysts. Cyst position depends on the relation with each gill arch. They commonly occur as a bulge at the front edge of the sternocleidomastoid, may or may not have a fistulous tract. As thyroglossal cysts, they can manifest at any age, but most often diagnosed in the first two decades of life, the diagnosis is more evident when their fistula is associated, even the first symptom may be superinfection of this conduit. Surgical treatment is required to resolve the infection before surgery.


 Another possibility is the differential diagnosis of carotid body tumor or carotid glomus, but in this case, there is no data discarded by vessels within the lesion when examined with Doppler.
