Endometrioma

Paciente femenina de 21 años de edad, Gestacion 0 Partos 0, Menarquia a los 11 años, Primera Relación Sexual a los 18 años ( infertilidad primaria 3 años ) con larga historia de Dismenorreas. Fue operada hace 2 años con el resultado de la extracción de nueve ( 9 ) miomas. Parece ser que le dieron esperanzas de que tras esa cirugía  iba a poder concebir. El examen sonografico trans-vaginal muestra Útero con presencia de pequeño nódulo miomatoso intramural de aprox 2,34 X 2,63 cm. El fondo de saco posterior muestra presencia de gran derrame hematico. El ovario izquierdo muestra imagen de quiste de paredes engrosadas e irregulares, con material grumoso fino, floculante ( compatible con derrame hematico ), la lesión mide aprox 6,90 X 6,44 X 6,44 cm, con un volumen aprox de 284,4 ml. Se concluye con los siguientes diagnósticos 1.-Endometrioma ovario izquierdo, 2.- Mioma Uterino intramural y 3.- Derrame Hematico en Fondo de Saco Posterior. En nuestra opinión  desde el punto de vista clínico  esta paciente desde su Menarquia padece de Endometriosis , la cual es la verdadera causante de su Dismenorrea y de su infertilidad primaria.

Female patient, 21 years old, gestation 0 births 0, menarche at age 11, First Sexual Intercourse at 18 years (primary infertility 3 years) with a long history of dysmenorrhea. She had surgery two years ago resulting in the removal of nine (9) myomas. It appears that it gave her hope that after this surgery would be able to conceive. The transvaginal sonographic examination shows the presence of the uterus with of small intramural fibroid nodule approx 2.34 X 2.63 cm. The posterior fornix shows the presence of a large spill blood count. The left ovary cyst image shows thickened and irregular walls, lumpy material fine flocculant (compatible with spill blood count), the cyst measures approx 6.90 X 6.44 X 6.44 cm, with a volume of 284.4 ml. It concludes with the following diagnoses 1.-Endometrioma left ovary, 2. - Uterine myoma intramural, and 3. - Spill Fund bloody in Rear Bag. In our opinion from the clinical point of view, this patient since menarche suffers from Endometriosis, which is the real cause of his dysmenorrhea and its primary infertility.
