Absceso

Masculino 55 años  con historia remota de trauma en cara posterior del tercio superior del antebrazo izquierdo. En dicha zona presenta tumefacción por lo cual le dio la impresión inicial al medico tratante de lipoma del área. El examen sonografico con transductor lineal de foco corto muestra presencia de área an-ecogena, contenido liquido con grumos finos en su interior y paredes internas irregulares. El área se encuentra rodeada de marcado engrosamiento epidérmico. Las medidas son de aprox: 2,71 X 1,83 X 0,81 cm, volumen de aprox: 2,1 c.c. El eco Doppler muestra aumento del flujo vascular en su área periférica - ver 2da imagen-. La elastografia muestra patrón tricolor con predominio del verde y el rojo,  por tanto, predominantemente áreas blandas, inflamatorias, score 2 Ueno. Dadas estas caracteristicas sonograficas concluimos con el diagnostico de hematoma abscesado de la cara posterior del antebrazo.

Abscess

Male 55 years with a remote history of trauma on the posterior aspect of the upper third of the left forearm. In that area, there is swelling that gave the initial impression to the treating doctor of lipoma of the area. The sonographic examination with a linear transducer of short-focus shows the presence of an anechoic area, liquid content with fine lumps inside, and irregular internal walls. The area is surrounded by marked epidermal thickening. The measurements are approx: 2.71 X 1.83 X 0.81 cm, volume of approx: 2.1 c.c. The echo-Doppler shows the increased vascular flow in its peripheral area - see 2nd image -. Elastography shows a tricolor pattern with a predominance of green and red, therefore, predominantly soft, inflammatory areas, score 2 of Ueno. Given these sonographic characteristics, we conclude with the diagnosis of abscessed hematoma of the posterior aspect of the forearm.
