Calculo en Uréter Distal

 Masculino 46 años de edad sin antecedentes personales de interés, viene por intensos dolores en fosa iliaca derecha, nauseas sin vómitos. Niega fiebre. Describe los dolores como intensos, insoportables, a pesar de haber tomado analgésicos. Disuria. Niega hematuria. El examen sonografico abdominal muestra leve dilatación hidronefrotica de riñón derecho(pelvis renal y sistema pielocalicial ) apreciándose algunas imágenes de cálculos intrarenales de pequeño tamaño. La vejiga  luce sonograficamente normal con paredes no engrosadas, sin presencia de pólipos, cálculos o ureteroceles. A nivel del meato urinario derecho a pocos milímetros de su salida a vejiga se visualiza imagen hiperecogenica ( cálculo ) de aprox: 5.7 mm en su eje mayor con sombra posterior. Aplicamos una técnica de Doppler Color en el área para visualizar los jets ureterales, de esta manera apreciamos ocho ( 8) jets ureterales izquierdos de forma consecutiva sin ningún jet ureteral derecho durante el tiempo de observación, concluimos por tanto, que existe una obstrucción distal del uréter derecho a nivel del meato urinario derecho.

Calculus in Distal Ureter

 A 46-year-old male with no personal history of interest presented with intense pain in the right iliac fossa, and nausea without vomiting. He denies fever. He describes the pain as intense, and unbearable, despite taking painkillers. Dysuria. He denies hematuria. Abdominal sonographic examination shows mild hydronephrotic dilatation of the right kidney (renal pelvis and pelvicalyceal system), showing some images of small intrarenal stones. The bladder appears sonographically normal with non-thickened walls, without the presence of polyps, stones, or ureteroceles. At the level of the right urinary meatus, a few millimeters from its outlet to the bladder, a hyperechoic image (calculus) of approx: 5.7 mm in its longest axis with a posterior shadow is displayed. We apply a Color Doppler technique in the area to visualize the ureteral jets, in this way we appreciate eight (8) left ureteral jets consecutively without any right ureteral jet during the observation time, we, therefore, conclude that there is a distal obstruction of the right ureter at the level of the right urinary meatus.
