📌 ANTECEDENTES / BACKGROUND

Femenina de 36 años, con antecedentes de colecistectomía y manguito gástrico. En tomografía abdominal previa se identificó una lesión hipodensa con realce arterial en el lóbulo derecho hepático. Presenta sobrepeso y antecedentes de diabetes familiar. Acude por náuseas y vómitos para evaluación sonográfica.

36-year-old female with a history of cholecystectomy and gastric sleeve surgery. A prior abdominal CT showed a hypodense liver lesion with arterial enhancement in the right lobe. She has an overweight status and a family history of diabetes. She presents with nausea and vomiting for abdominal ultrasound evaluation.

📌 HALLAZGOS SONOGRÁFICOS / SONOGRAPHIC FINDINGS

El hígado se encuentra aumentado de tamaño (16.2 cm). Se identifica un nódulo hiperecogénico de bordes ligeramente irregulares, de aprox. 3.89 × 3.52 cm, localizado en el lóbulo derecho. El parénquima hepático adyacente muestra aumento difuso de la ecogenicidad. La vesícula biliar no es visualizada (postcolecistectomía). No se observan dilataciones ni cálculos en la vía biliar.

The liver is enlarged (16.2 cm). A mildly irregular hyperechoic nodule measuring 3.89 × 3.52 cm is identified in the right lobe. The adjacent hepatic parenchyma shows increased echogenicity. The gallbladder is not visualized (post-cholecystectomy). No biliary dilation or stones are seen.

📌 CONCLUSIÓN / CONCLUSION

ES:

Nódulo hepático hiperecogénico de ~3.8 cm con bordes ligeramente irregulares, flujo periférico leve y rigidez solo discretamente mayor que el tejido circundante. El conjunto de hallazgos sugiere una lesión probablemente benigna, con posibilidades como adenoma hepático, hemangioma atípico o FNH poco típica.

Dado el antecedente de realce arterial en tomografía, se recomienda correlación con TC o RM multiphasica para caracterización definitiva.

No se observan signos de colestasis ni hipertensión portal.

EN:

Hyperechoic hepatic nodule (~3.8 cm) with mildly irregular margins, mild peripheral flow, and stiffness only slightly above the adjacent parenchyma. Findings favor a likely benign lesion, with possibilities including hepatic adenoma, atypical hemangioma, or subtle FNH.

Given the prior arterial enhancement on CT, correlation with multiphasic CT or MRI is recommended for definitive characterization.

No evidence of cholestasis or portal hypertension.
