Una Situación Bizarra
Ningún otro titular me ha parecido mas adecuado para describir lo que acaba de sucederme por vez primera en mi ya larga carrera como medico y como sonografista.
Recibo un paciente para realizarle una sonografia trans-rectal de Próstata. Desde que entra por la puerta me llama la atención su estatura y corpulencia, rostro ampuloso, con pómulos salientes y ojos hundidos, voz ronca y fuerte cuando contesta su edad : 59 anos.Tras colocarse en la posición de decubito lateral izquierdo, le pregunto, como es habitual, las razones por las cuales le indican este examen, me contesta diciendo que era porque tenia problemas renales con dificultad para la micción , el urologo había tratado de ponerle una sonda vesical , algo que no pudo realizar ( no explicó las razones ).
Al introducir la sonda por el Recto, rápidamente me percato que no tengo a la vista ni la Próstata ni las Vesículas Seminales, debido a esto realizo diversas maniobras para tratar de localizar esos órganos, como mis esfuerzos eran infructuosos, decidí ( en un acto de lucidez mental que suelo describir como " se me encendió una bombilla en mi cerebro ") , le pedí al paciente que se pusiese en decubito supino ( boca arriba ) y ahí descubrí la causa del fracaso de mi exploración. Con gran sorpresa se desplegó ante mi un Clítoris gigante, una mini-bolsa escrotal vacía de contenido y unos labios mayores sin apertura vaginal. Resumiendo la situación se trataba de una persona con un estado Inter- Sexual. Lo único que se me ocurrió preguntar era si había tenido relaciones sexuales alguna vez......Me contesto de una manera muy brusca diciendo....No, claro que no. Debido a su actitud ofendida decidí no preguntar nada mas para respetar su condición de ser humano que ha llevado toda su vida una carga emocional-psicológica muy pesada, por la misma razón no quise tomar fotos de los genitales ambiguos de este paciente.
Por favor no me pregunten las razones por las cuales el Urológo mandó a hacer ese tipo de exploración.....

I get a patient to perform transrectal sonography of the prostate. From that walks through the door strikes me your height and build, pompous face with high cheekbones and sunken eyes, hoarse voice and strong when answering his age: 59 years. Having placed in the left lateral decubitus position, I ask, as usual, the reasons why this test tells me answers saying it was because he had kidney problems with urination difficulty, the urologist had tried to put a urinary catheter, something he could not make (not explains the reasons).
When inserting the probe in the Rectum, I realize quickly that I have not the views Seminal Vesicles or Prostate nor because it performed various maneuvers to try to locate these bodies, as my efforts were fruitless, I decided (in an act of alertness that usually describes as "I lit a light bulb in my brain"), I asked the patient to stand in supine (face up) and there I discovered the cause of the failure of my exploration. With great surprise unfolded before me a giant clitoris, a mini-empty scrotum, and contents without opening a vaginal labia. Summarizing the situation was a person with an Inter-Sexual state. The only thing I could think to ask was whether he had ever had sex ...... I replied in a very abrupt saying... No, of course not. Because of his attitude offended I decided not to ask anything more to respect their human condition that his life has been an emotional-psychological heavy, for the same reason I did not take pictures of this patient's ambiguous genitalia.
Please do not ask me the reasons why the control Urologist to do that kind of exploration .....
