Masculino 67 años de edad, diabético insulino-dependiente con síntomas de impotencia desde hace un año. Desde hace 1-2 meses se queja de molestias en testículo derecho y dice que ocasionalmente tiene dolores no muy intensos. El examen sonografico con transductor lineal de 10.0 MHz muestra la presencia de nódulo solido hipo ecogénico en testículo derecho, mide aprox: 0.42 X 0.35 cm, muestra pequeño flujo periférico al aplicar el Doppler Color. Con la Elastografia se demuestra una relacion B/A menor de 1 (0.57) lo cual sugiere lesión benigna.
El testículo mide aprox: 1.41 X 2.69 X 1.23 cm con volumen aprox: de 2.45 c.c. El epidídimo luce sonograficamente normal, no se detectan signos de varicocele en plexo pampiniforme derecho. El testículo izquierdo muestra ecogenicidad y homogeneidad normales, mide aprox: 1.42 X 2.11 X 1.14 cm con volumen aprox: de 1.79 c.c. No se detectan lesiones quísticas o solidas. El epidídimo muestra área anecogena, quística de aprox: 1.33 X 0.96 cm. La bolsa escrotal muestra presencia de líquido claro, sin grumos, con volumen aprox; de 9.73 c.c. No se detectan signos sonograficos de varicocele izquierdo. Se concluye con los diagnósticos de Nódulo Solido Testículo Derecho, probablemente benigno a la espera de confirmación con otros métodos diagnósticos, Quiste Epidídimo e Hidrocele Izquierdo.
Testicular Tumor & Hydrocele & Epididymal Cyst
67-year-old male, insulin-dependent diabetic with symptoms of impotence for a year. For 1-2 months he has been complaining of discomfort in the right testicle and says that occasionally he has pain that is not very intense. The sonographic examination with a 10.0 MHz linear transducer shows the presence of a hypoechogenic solid nodule in the right testicle, measuring approximately: 0.42 X 0.35 cm, showing small peripheral flow when applying Color Doppler. Elastography shows a B/A ratio of less than 1 (0.57), which suggests a benign lesion. The testicle measures approx: 1.41 X 2.69 X 1.23 cm with a volume of approx: 2.45 c.c. Sonographically, the epididymis appears normal, and no signs of varicocele are detected in the right pampiniform plexus. The left testicle shows normal echogenicity and homogeneity, measuring aapproximately1.42 X 2.11 X 1.14 cm with a volume of aapproximately1.79 c.c. No cystic or solid lesions were detected. The epididymis shows an anechogenic, cystic area of approx: 1.33 X 0.96 cm. The scrotal bag shows the presence of clear fluid, without lumps, with an approximate volume; of 9.73 c.c. Sonographic signs of the left varicocele are not detected. It concludes with tight Testicular Solid Nodule, diagnoses probably benign pending confirmation with other diagnostic methods, Epididymal Cyst, and Left Hydrocele.
