Epididimitis Aguda en Niño

Masculino 07 años de edad que un dia previo a este examen presentó fiebre alta que remitió con antipireticos. Al dia siguiente sus padres comenzaron a notar que cojeaba y se quejaba de dolor en testiculo izquierdo. Al examen presenta enrojecimiento del testiculo y se rehusa a ser examinado ya que su pediatra cuando lo examinó le produjo mucho dolor. Tras prometerle que no le produciria dolor al realizar la sonografia comenzamos el examen. El diagnostico de impresión previo de su pediatra era torsión testicular pero a nosotros nos llamó la atención la buena motricidad previa al examen sonografico que mostraba el paciente lo cual nos hizo pensar en otra patologia, no obstante, fue una sorpresa que al comenzar el examen, lo que se nos mostró fue un epididimo izquierdo marcadamente engrosado,edematoso. Al Doppler Color  se aprecia flujo normal del testiculo izquierdo lo cual descarta la torsión testicular.El grosor del epidimo izquierdo es de 1.16 cm en contraste con el epididimo derecho que era de 0,46 cm.Tambien se apreció un pequeño hidrocele izquierdo probablemente relacionado con el proceso inflamatorio local.

Acute Epididymitis in Child

A 7-year-old male who one day prior to this examination presented a high fever that remitted with antipyretics. The next day his parents began to notice that he was limping and complaining of pain in his left testicle. Upon examination, he presented redness of the testicle and refused to be examined since his pediatrician when he examined it caused him a lot of pain. After promising that it would not cause pain when performing the sonography we began the examination. His pediatrician's previous impression diagnosis was testicular torsion, but we were struck by the good motor skills that the patient showed prior to the sonographic examination, which made us think of another pathology, however, it was a surprise that when the examination began, what was shown to us was a markedly thickened, edematous left epididymis. Color Doppler showed a normal flow of the left testicle which rules out testicular torsion. The thickness of the left epididymis is 1.16 cm in contrast to the right epididymis, which was 0.46 cm. A small left hydrocele was also seen, probably related to the local inflammatory process.
