Quiste Epidermico-Sebaceo & Quiste Dérmico Abscesado

 Paciente femenina de 53 años de edad que presenta área nodular, blanda, alargada , en zona superficial de la fosa iliaca derecha por encima de la cresta iliaca. No es dolorosa a la palpación. Dice que lo tiene desde ´hace tiempo´. El examen sonográfico con transductor linear de 10.0 MHz demuestra la presencia de área hipo ecogénica  a nivel de la dermis-tejido celular subcutáneo de la zona, muestra un aspecto fusiforme con una especie de canal fistuloso dirigido hacia la piel. Su interior luce lleno de material grumoso fino, floculante, con pobre refuerzo ecogénico posterior y las paredes lucen ser regulares, algo engrosadas. La lesión mide aprox: 1.73 X 1.58 X 0.75 cm, volumen aprox: 1.07 c.c. El eco Doppler de la lesión muestra ligero aumento del flujo en la parte mas superficial de la lesión ( ver 1ra foto).La Elastografia demuestra un patrón de color con score 2 de Ueno y Strain Ratio de 1.35, ambos datos demostrativos de la benignidad de la lesión. Se concluye con el diagnostico de Quiste Dérmico Abscesado.

Abscessed Dermal Cyst

A 53-year-old female patient presented with an elongated, soft nodular area in the superficial area of ​​the right iliac fossa above the iliac crest. She is not painful on palpation. She says she has had it for 'a long time'. The sonographic examination with a 10.0 MHz linear transducer shows the presence of a hypoechogenic area at the level of the dermis-subcutaneous cellular tissue of the area, it shows a fusiform appearance with a kind of fistulous canal directed towards the skin. Its interior appears full of fine, flocculating lumpy material, with poor posterior echogenic reinforcement, and the walls appear to be regular, somewhat thickened. The lesion measures approx: 1.73 X 1.58 X 0.75 cm, volume approx: 1.07 c.c. The Doppler echo of the lesion shows a slight increase in flow in the most superficial part of the lesion (see 1st photo). Elastography shows a color pattern with an Ueno score of 2 and a Strain Ratio of 1.35, both data demonstrating the benignity of injury. It concludes with the diagnosis of an Abscessed Dermal Cyst.

La misma paciente del caso anterior muestra lesión nodular de color negro en la base izquierda del cuello. Dice que comenzó como una espinilla pequeña y que, al presionarlo sale un liquido blanco-amarillento de olor muy penetrante y desagradable. Lleva ´mucho tiempo ´con esto. Al examen sonográfico se visualiza a nivel dérmico un nódulo solido, hipo ecogénico, de limites regulares, sin refuerzo o sombra ecogénica posterior, mide aprox: 1.52 X 1.59 X 0.85 cm. El eco-Doppler es negativo y la Elastografia presenta un score 2 de Ueno con Strain Ratio nódulo/ grasa de 11.05 compatible con lesión sospechosa de malignidad. Se concluye con el diagnostico de Quiste Sebáceo.

Sebaceous Cyst

The same patient from the previous case shows a black nodular lesion on the left base of the neck. She says that she started out as a small pimple and that when pressed, a yellowish-white liquid with a very pungent and unpleasant odor comes out. She's been 'a long time' with this. On sonographic examination, a solid, hypoechogenic nodule with regular limits is visualized at the dermal level, without reinforcement or posterior echogenic shadow, measuring approx: 1.52 X 1.59 X 0.85 cm. The echo-Doppler is negative and the Elastography presents an Ueno score 2 with Strain Ratio nodule / fat of 11.05 compatible with a lesion suspected of malignancy. It concludes with the diagnosis of Sebaceous Cyst
