Osteomielitis & Absceso Pretibial

 Masculino 18 años edad con antecedente lejano de fractura de tendón patelar en rodilla izquierda, esta lesión se produjo jugando fútbol. Hace dos semanas tuvo una caída con erosión superficial en el área pre-tibial izquierda casi a la altura de la rodilla izquierda. En el área se visualiza tumefacción con enrojecimiento ,aumento de la temperatura local y sensación de pequeña fluctuación al palpar el área. En el centro del foco se aprecia un punto negro. El examen sonografico con transductor lineal de 10.0 MHz muestra imagen anecogena de paredes internas irregulares, anfractuosas con un contenido de grumos finos floculantes, el área mide aprox: 2.80 X 2.60 X 2.48 cm con volumen de aprox: 9.45 c.c.. En la superficie ósea subyacente ( tibia) se visualiza imagen de discontinuidad ósea al través de la cual se aprecia el flujo del material grumoso ( purulento) .El Doppler color muestra flujo vascular en la base de la lesión. Se concluye con los diagnósticos de Absceso pre-tibial y Osteomielitis de la Tibia Subyacente. 

Osteomyelitis & Pretibial Abscess

 An 18-year-old male with a history of distant antecedent of patellar tendon fracture in the left knee, this injury occurred while playing soccer. Two weeks ago he had a fall with superficial erosion in the left pre-tibial area almost at the level of the left knee. In the area, there is swelling with redness, an increase in local temperature, and a sensation of small fluctuation when palpating the area. A black dot is visible in the center of the focus. The sonographic examination with a 10.0 MHz linear transducer shows an anechoic image of irregular, anfractuous internal walls with the content of fine flocculating lumps, the area measures approx: 2.80 X 2.60 X 2.48 cm with a volume of approx: 9.45 cc. On the underlying bone surface (tibia), an image of bone discontinuity is visualized through which the flow of the lumpy (purulent) material can be seen. Color Doppler shows vascular flow at the base of the lesion. It concludes with the diagnoses of Pre-tibial Abscess and Osteomyelitis of the Underlying Tibia
