Hidranencefalia

Femenina 25 años edad G2 C1 cursando embarazo de 36.6 semanas por FUM. Viene referida de un centro público donde al realizarle una sonografía le reportaron una hidrocefalia severa. El examen sonografico muestra producto único, cefálico con un polo cefálico de dimensiones gigantescas (Diámetro Biparietal (DBP) de 13.7 cm, Diámetro occipito-frontal (OFD) de 18.3 cm y una circunferencia craneal (HC) de 48.6 cm. Debido a las dimensiones del cráneo fetal no fue posible utilizarlo para el cálculo de la edad gestacional, para cumplir esa función tuvimos que recurrir a la longitud del femur y a la fecha de la última menstruación ( FUM).El resto de las medidas corporales resultaron dentro de lo esperado para la edad gestacional si exceptuamos su cálculo de peso corporal que demostró una macrosomía importante de 9.11 libras. El líquido amniótico estaba aumentado (polihidramnios leve) con abundantes grumos finos internos. El perfil biofísico resulto en 4/8 puntos y el estudio Doppler es compatible con un tipo II con importante déficit circulatorio a nivel cerebral. Se concluye con el diagnostico de Hidranencefalia severa en embarazo de 36 semanas con polihidramnios leve y con producto macrosómico con un perfil biofísico de 4 sobre 8 y con déficit circulatorio a nivel de circulación craneal.

La Hidranencefalia es una situación no compatible con la supervivencia postparto, suelen morir antes de un año por lo que se plantea el aborto terapéutico que dada la situación está altamente justificado.

Los hallazgos sonograficos son los siguientes:

-Tejidos corticales ausentes,

-Islas de tejido residual en polo occipital o en región orbitofrontal,

-Tejido medial temporal irrigado por circular basilar,

-Tálamo y fosa posterior preservados,

-Ausencia arterias cerebrales mediales.

Hydranencephaly

25-year-old female G2 C1 undergoing a pregnancy of 36.6 weeks due to FUM. She is referred from a public center where, when an ultrasound was performed, they reported severe hydrocephalus. Due to the dimensions of the fetal skull, it was not possible to use it to calculate gestational age. To fulfill this function, we had to resort to the length of the femur and the date of the last menstruation (LMP). The rest of the Body measurements were within what was expected for gestational age if we excepted his calculation of body weight which showed significant macrosomia of 9.11 pounds Amniotic fluid was increased (mild polyhydramnios) with abundant fine internal lumps The biophysical profile was 4/8 points and the Doppler study is compatible with a type II with the significant circulatory deficit at the brain level. It is concluded with the diagnosis of severe hydranencephaly in a 36-week pregnancy with mild polyhydramnios and macrosomic product with a biophysical profile of 4 out of 8 and with deficit circulation at the level of the cranial circulation.

Hydranencephaly is a situation that is not compatible with postpartum survival, they usually die within a year, so therapeutic abortion is considered, which given the situation is highly justified.

The sonographic findings are as follows:

-Absent cortical tissues,

-Islands of residual tissue in the occipital pole or in the orbitofrontal region,

-Temporal medial tissue supplied by circular basilar,

-Thalamus and posterior fossa preserved,

-Absence of medial cerebral arteries.
