Masculino 68 años de edad, antecedentes de Hipertensión Arterial Sistémica con historia de cuatro meses de dolor testicular derecho que se ha ido incrementando con el tiempo junto a aumento del tamaño de la bolsa escrotal. El urólogo lo envió a chequeo sonografico con sospecha de tumor testicular derecho. El examen sonografico muestra testículo derecho aumentado de tamaño con volumen aprox: de 16.50 ml. En comparación el volumen testicular izquierdo es de 5.60 ml. El testículo derecho no muestra lesiones focales parenquimatosas, pero si se aprecia un aumento significativo del flujo vascular al aplicar el Doppler Color (ver fotos No 1-2 ). El estudio con Elastografia del testiculo derecho muestra un patron de color con score 2 de Ueno,elastografia de concha (Shell) de 0.79 y Strain Ratio B/A de 0.15 ,datos todos compatibles con situacion de benignidad (fotos 3-4).Ambos epididimos muestran quistes politabicados con contenido grumoso fino (hematico)-ver fotos # 6-7). En la fotos #6 apreciamos hidrocele no tabicado en bolsa escrotal derecha con volumen aprox: de 15.80 c.c.No se encontraron datos sonograficos de varicoceles.Se concluye con los diagnosticos de a) Orquitis Derecha,b) hidrocele derecho y c) quistes hematicos de ambos epididimos.
Orchitis & Hydrocele & Bilateral Hematic Cysts of Epididymis
68-year-old male, with a history of Systemic Arterial Hypertension with a four-month history of right testicular pain that has been increasing over time together with an increase in the size of the scrotal sac. The urologist sent him for a sonographic checkup with suspicion of a right testicular tumor. The sonographic examination shows an enlarged right testicle with a volume of approx: 16.50 ml. In comparison, the left testicular volume is 5.60 ml. The right testicle does not show focal parenchymal lesions, but a significant increase in vascular flow can be seen when applying Color Doppler (see photos No 1-2). The Elastography study of the right testicle shows a color pattern with an Ueno score of 2, Shell elastography of 0.79, and Strain Ratio B/A of 0.15, data all compatible with a situation of benignity (photos 3-4). Both epididymides show polytabulate cysts with fine lumpy content (hematic)-see photos # 6-7). In photo #6 we can see hydrocele not septated in the right scrotal bag with a volume of approximately: 15.80 cc. No sonographic data of varicoceles were found. It concludes with the diagnoses of a) Right Orchitis, b) right hydrocele, and c) hematic cysts of both epididymis.
