Tiroiditis Hashimoto & Nódulo Mixto Tiroides

Femenina 43 años, obesa, diagnosticada desde hace mas de 4 años de nodulo tiroideo. Le fue realizada una biopsia con resultados negativos para malignidad. En la actualidad los datos analíticos de T3 y T4 resultan en valores normales pero los anticuerpos antitiroideos permanecen elevados. El examen sonografico del tiroides muestra un aumento global del tamaño glandular con engrosamiento del istmo que mide 6.9 mm ( normal hasta 5 mm ). Los limites glandulares lucen irregulares y se aprecia heterogeneidad focal diseminada con múltiples áreas focales hipoecogenicas. El lóbulo derecho muestra imagen nodular solida con área lacunar central (aspecto de Donut), mide aprox: 2.02 X 1.05 cm y muestra aumento del flujo vascular periférico al aplicar el Doppler Color. La Elastografia del nodulo muestra un score 2 de Ueno y un Strain Ratio de 0.42, compatible con lesión benigna. El ACR-TI-RADS es de 01 puntos, por lo tanto,no sospechoso. Se concluye con los diagnosticos de Tiroiditis Crónica de Hashimoto con nodulo mixto tiroides de caracter benigno. Se valora la importancia de la Elastografia para clasificar con precision la benignidad-malignidad de una lesion solida tiroidea.

Hashimoto's Thyroiditis & Mixed Thyroid Nodule

Female, 43 years old, obese, diagnosed for more than 4 years with a thyroid nodule. She underwent a biopsy with negative results for malignancy. Currently, the analytical data for T3 and T4 are standard, but antithyroid antibodies remain elevated. The sonographic thyroid examination shows a global increase in glandular size with thickening of the isthmus that measures 6.9 mm (normal up to 5 mm). The glandular boundaries appear irregular and there is widespread focal heterogeneity with multiple focal hypoechoic areas. The right lobe shows a solid nodular image with a central lacunar area (donut appearance), measures approx: 2.02 X 1.05 cm and shows increased peripheral vascular flow when applying Color Doppler. Elastography of the nodule shows an Ueno score of 2 and a Strain Ratio of 0.42, compatible with a benign lesion. The ACR-TI-RADS is 01 points, therefore not suspicious. It concludes with the diagnosis of Chronic Hashimoto's Thyroiditis with a benign mixed thyroid nodule. The importance of elastography to accurately classify benignity-malignancy of a solid thyroid lesion is valued.
