Masculino 73 años de edad, sin antecedentes familiares de cáncer de próstata viene por segundo año consecutivo a chequeo prostático. Dice tener nicturia de 2-3 veces cada noche. Su Antígeno Prostático Especifico (PSA) se reportó ligeramente por encima de 4, pero al interrogar al paciente declaro que había tenido actividad sexual un día antes de la toma de la muestra sanguínea, lo cual invalida ese resultado. El examen sonografico ,vía transrectal, muestra vesículas seminales con aspecto, forma, ecogenicidad, homogeneidad y tamaño normales. La próstata luce aumentada de tamaño con un volumen de aprox: 44.9 gramos compatible con Hipertrofia Prostática grado II lo cual es un aumento de tamaño aceptable para la edad del paciente. En la periferia del lóbulo izquierdo se visualiza un pequeño nódulo sólido, hipo ecogénico, ya visualizado y analizado en el examen prostático del año anterior. El nódulo mide aprox:1.44 X 0.93 cm. Al aplicar el Doppler Color se aprecia flujo interno. La Elastografia muestra un Score 3 de Ueno y al utilizar el Strain Ratio aplicado al nódulo en comparación con el tejido prostático normal muestra un valor de 1.00, compatible con lesión benigna. Con ello se demuestra la utilidad de la Elastografia para valorar la peligrosidad o no de una lesión nodular periférica de un nódulo prostático. Se concluye con los diagnósticos de Hipertrofia Prostática grado II y Nódulo Solido Prostático Benigno.
Prostatic nodule
A 73-year-old male, with no family history of prostate cancer, comes for the second consecutive year for a prostate check-up. He claims to have nocturia 2-3 times each night. His Prostate Specific Antigen (PSA) was reported to be slightly above 4, but when questioning the patient he stated that he had been sexually active one day before the blood sample was taken, which invalidates that result. Transrectal sonographic examination shows seminal vesicles with normal appearance, shape, echogenicity, homogeneity, and size. The prostate appears enlarged with a volume of approx: 44.9 grams compatible with grade II Prostatic Hypertrophy, which is an acceptable size increase for the patient's age. In the periphery of the left lobe, a small solid, hypo-echogenic nodule was visualized, already visualized, and analyzed in the prostate exam of the previous year. The nodule measures approx: 1.44 X 0.93 cm. When applying the Color Doppler internal flow is appreciated. Elastography shows an Ueno Score 3 and when using the Strain Ratio applied to the nodule in comparison with normal prostate tissue shows a value of 1.00, compatible with a benign lesion. This demonstrates the usefulness of Elastography to assess the danger or not of a peripheral nodular lesion of a prostatic nodule. It concludes with the diagnoses of Prostatic Hypertrophy grade II and Benign Prostatic Solid Nodule.
