Masculino 62 años de edad con historia de aparición de dolor súbito en testículo derecho con aumento de tamaño del mismo desde hace 4 semanas, al levantarse de la cama. Fue tratado con antibioticoterapia y con antinflamatorios con lo cual mejoró ligeramente, sin desaparecer el dolor o la hinchazón del escroto/testículo. El examen sonografico testicular con transductor linear de 10.5 MHz muestra un testículo hipo ecogénico con cierto grado de heterogeneidad focal diseminada. Luce aumentado de tamaño con un volumen de aprox: de 12.28 c.c., (en comparación el testículo izquierdo que muestra un volumen de 4.74 c.c.). Junto a esto se aprecia un engrosamiento significativo de la pared de la bolsa escrotal derecha con 0.85 cm de grosor. Muestra flujo Doppler normal en su parénquima lo cual excluye la torsión testicular. No se detectan signos sonograficos de hidrocele. Ambos epidídimos lucen normales, las cabezas de estos miden aprox: 0.46 cm la derecha y 0.43 cm la izquierda. Se concluye con los diagnósticos de Orquitis y Edema Crónico de Escroto Derecho.
Orchitis and Chronic Scrotal Edema
A 62-year-old male with a history of sudden pain in the right testicle with an increase in its size for 4 weeks, when getting out of bed. He was treated with antibiotic therapy and anti-inflammatories, with which he improved slightly, without disappearing the pain or swelling of the scrotum/testicle. Testicular sonographic examination with a 10.5 MHz linear transducer shows a hypoechogenic testicle with a certain degree of disseminated focal heterogeneity. It looks enlarged with a volume of approx: 12.28 c.c., (compared to the left testicle which shows a volume of 4.74 c.c.). Along with this, significant thickening of the wall of the right scrotal bag with a thickness of 0.85 cm can be seen. It shows normal Doppler flow in its parenchyma, which excludes testicular torsion. Sonographic signs of hydrocele are not detected. Both epididymides look normal, their heads measure approximately: 0.46 cm on the right and 0.43 cm on the left. It concludes with the diagnoses of Orchitis and Chronic Edema of the Right Scrotum.
