Paciente femenina 68 años de edad viene aterrorizada por el temor generado al decirle su ginecólogo que visualizó en su cuello uterino signos sospechosos de infección por el virus del papiloma humano. El examen sonografico demostró un útero en anteversión, de pequeño tamaño, hipotrófico, en relacion con el status postmenopáusico de la paciente. Mide aprox: 5.36 X 2.65 X 1.73 cm. No hay signos sonograficos de lesiones miomatosas. El endometrio es fino (2.1 mm) sin mostrar signos sospechosos de lesiones. El fondo de saco posterior de Douglas es sonograficamente normal sin presencia de líquido. El cérvix luce cerrado, muestra flujo Doppler negativo y la Elastografia mostró un score de color 2-3 de Ueno, compatible con cérvix duro mostrando un Strain Ratio B/A menor de 1 (0.21) compatible con negatividad para lesión maligna. Con este estudio valoramos el estatus del cuello uterino para diferenciar cuellos uterinos sin patologías malignas prescindiendo del uso de la biopsia y valoramos su utilidad junto al examen clásico del Papanicolau para estudiar estos casos.
Assessment of the Elastography status of the Cervix in a Menopausal Patient
A 68-year-old female patient is terrified by the fear generated by her gynecologist telling her that he saw suspicious signs of human papillomavirus infection on her cervix. Sonographic examination revealed a small, hypotrophic, anteverted uterus, in relation to the patient's postmenopausal status. Measures approx: 5.36 X 2.65 X 1.73 cm. There are no sonographic signs of myomatous lesions. The endometrium is thin (2.1 mm) without showing suspicious signs of lesions. The posterior fornix of Douglas is sonographically normal with no presence of fluid. The cervix appears closed, and shows negative Doppler flow and Elastography showed an Ueno color score of 2-3, compatible with a hard cervix showing a B/A Strain Ratio of less than 1 (0.21) compatible with negativity for the malignant lesion. With this study, we assess the status of the cervix to differentiate cervixes without malignant pathologies, dispensing with the use of biopsy, and we assess its usefulness together with the classic Papanicolaou exam to study these cases.
