SONOGRAFÍA DE TIROIDES CON DOPPLER COLOR Y ELASTOGRAFÍA SHEAR WAVE


ANTECEDENTES CLÍNICOS

Paciente masculino de 65 años de edad, con antecedente de hemi-lobectomía izquierda del tiroides realizada aproximadamente 4 años atrás. Se encuentra bajo tratamiento médico por su condición tiroidea y acude para evaluación sonográfica de control.

HALLAZGOS SONOGRÁFICOS

Se identifica ausencia quirúrgica del lóbulo tiroideo izquierdo, en relación con antecedente de hemi-lobectomía.

El istmo tiroideo remanente se encuentra marcadamente engrosado, con espesor aproximado de 13.4 mm (valor de referencia habitual ≤5 mm), presentando contornos discretamente irregulares y marcada disminución difusa de la ecogenicidad.

En el extremo izquierdo del istmo se identifica imagen nodular de composición mixta sólido-quística, de aproximadamente 1.68 × 1.40 cm, que deberá ser valorada de acuerdo con sus características ecográficas individuales para establecer su categoría de riesgo.

El lóbulo tiroideo derecho se encuentra aumentado de tamaño, con contornos discretamente irregulares y ecogenicidad globalmente disminuida.

Sus dimensiones son aproximadamente:

  • Longitud: 5.33 cm
  • Diámetro AP: 2.82 cm
  • Diámetro transversal: 3.90 cm
  • Volumen estimado: 30.7 cc

El parénquima tiroideo remanente presenta aspecto heterogéneo e hipoecogénico, hallazgos que, en conjunto con el antecedente clínico, son compatibles con tiroiditis crónica autoinmune tipo Hashimoto.

DOPPLER COLOR

El estudio Doppler color demuestra disminución global de la vascularización del parénquima tiroideo remanente, sin evidencia de incremento difuso significativo del flujo.

ELASTOGRAFÍA SHEAR WAVE (SWE)

Se realizó elastografía Shear Wave sobre el parénquima tiroideo del lóbulo derecho.

Se obtuvieron 10 mediciones técnicamente válidas, con los siguientes resultados:

  • Mediana de rigidez: 35.30 kPa
  • Velocidad de transmisión: 3.39 m/s

Estos valores evidencian un incremento marcado de la rigidez del parénquima tiroideo, hallazgo que puede observarse en relación con cambios fibróticos avanzados asociados a tiroiditis crónica de Hashimoto.

La interpretación de los valores de SWE debe realizarse en conjunto con los hallazgos morfológicos, Doppler y contexto clínico, debido a que los valores de corte pueden variar según el equipo, técnica de adquisición y características del paciente.

CONCLUSIONES

  1. Cambios sonográficos difusos del tejido tiroideo remanente, caracterizados por aumento de volumen, heterogeneidad e hipoecogenicidad, asociados a disminución global de la vascularización al Doppler color, hallazgos compatibles con tiroiditis crónica autoinmune (enfermedad de Hashimoto).
  2. Marcado incremento de la rigidez del parénquima tiroideo mediante SWE, con mediana de 35.30 kPa (3.39 m/s), “marcado incremento de la rigidez, compatible con importante componente fibrótico/posible fibrosis avanzada en el contexto de Hashimoto” 
  3. Nódulo tiroideo de composición mixta sólido-quística, localizado en el extremo izquierdo del istmo, de 1.68 × 1.40 cm. Se recomienda caracterización ecográfica completa de sus componentes, márgenes, orientación, ecogenicidad y presencia/ausencia de calcificaciones para establecer su categoría de riesgo y determinar la conducta apropiada.
  4. Estado post hemi-lobectomía izquierda.

CORRELACIÓN CLÍNICA-LABORATORIO

Se recomienda correlacionar los hallazgos sonográficos con TSH, T4 libre y anticuerpos antitiroideos (especialmente anti-TPO), además de los antecedentes y tratamiento actual del paciente.


LEYENDAS DE LAS IMÁGENES

1. Istmo tiroideo engrosado: imagen longitudinal/transversal que demuestra marcado engrosamiento del istmo, con espesor aproximado de 13.4 mm y ecogenicidad disminuida.

2. Lóbulo tiroideo derecho: aumento de volumen y alteración difusa de la ecogenicidad del parénquima tiroideo remanente.

3. Volumen del lóbulo derecho: mediciones longitudinal, AP y transversal utilizadas para estimar un volumen aproximado de 30.7 cc.

4. Lesión mixta istmo-lóbulo tiroideo: imagen nodular de composición sólido-quística, localizada hacia el extremo izquierdo del istmo, de aproximadamente 1.68 × 1.40 cm.

5. Doppler color del lóbulo derecho: disminución global de la vascularización del parénquima tiroideo.

6. Elastografía SWE del lóbulo derecho: mapa de rigidez tisular obtenido mediante Shear Wave Elastography.

7. Valores de elastografía SWE: 10 mediciones válidas con mediana de 35.30 kPa y velocidad de transmisión de 3.39 m/s, evidenciando marcada elevación de la rigidez del tejido tiroideo.


THYROID ULTRASOUND WITH COLOR DOPPLER AND  

                     SHEAR WAVE ELASTOGRAPHY

CLINICAL HISTORY

65-year-old male patient with a history of left thyroid hemilobectomy performed approximately 4 years ago. The patient is currently receiving medical treatment for his thyroid condition and presents for routine sonographic follow-up.

SONOGRAPHIC FINDINGS

Surgical absence of the left thyroid lobe is identified, consistent with the patient's history of hemilobectomy.

The remaining thyroid isthmus is markedly thickened, measuring approximately 13.4 mm in thickness (usual reference value ≤5 mm), with mildly irregular contours and marked diffuse reduction in echogenicity.

At the left aspect of the isthmus, a mixed solid-cystic nodular lesion is identified, measuring approximately 1.68 × 1.40 cm. This lesion should be further characterized according to its individual sonographic features in order to establish its appropriate risk category.

The right thyroid lobe is enlarged, with mildly irregular contours and globally decreased echogenicity.

Its approximate measurements are:

  • Length: 5.33 cm
  • AP diameter: 2.82 cm
  • Transverse diameter: 3.90 cm
  • Estimated volume: 30.7 cc

The remaining thyroid parenchyma is heterogeneous and hypoechoic. Taken together with the clinical history, these findings are compatible with chronic autoimmune thyroiditis (Hashimoto's thyroiditis).

COLOR DOPPLER

Color Doppler examination demonstrates globally decreased vascularity of the remaining thyroid parenchyma, with no evidence of significant diffuse hypervascularity.

SHEAR WAVE ELASTOGRAPHY (SWE)

Shear Wave Elastography was performed on the thyroid parenchyma of the right lobe.

A total of 10 technically valid measurements were obtained, with the following results:

  • Median tissue stiffness: 35.30 kPa
  • Shear wave velocity: 3.39 m/s

These values demonstrate a marked increase in thyroid parenchymal stiffness, a finding that may be associated with advanced fibrotic changes in the setting of chronic Hashimoto's thyroiditis.

CONCLUSIONS

  1. Diffuse sonographic changes involving the remaining thyroid tissue, characterized by enlargement, heterogeneity, and hypoechogenicity, associated with globally decreased vascularity on Color Doppler, findings compatible with chronic autoimmune thyroiditis (Hashimoto's disease).
  2. Markedly increased thyroid parenchymal stiffness on SWE, with a median value of 35.30 kPa (3.39 m/s), compatible with a significant fibrotic component / possible advanced fibrosis in the setting of Hashimoto's thyroiditis.
  3. Mixed solid-cystic thyroid nodule located at the left aspect of the isthmus, measuring 1.68 × 1.40 cm. Complete sonographic characterization of its composition, margins, orientation, echogenicity, and presence or absence of calcifications is recommended in order to establish the appropriate risk category and management.
  4. Status post left thyroid hemilobectomy.

CLINICAL AND LABORATORY CORRELATION

Correlation with TSH, free T4, and thyroid autoantibodies (particularly anti-TPO antibodies) is recommended, together with the patient's clinical history and current treatment.
