De lo Complejo a lo Simple

En ocasiones las cosas se invierten, caminan al revés como los cangrejos, que además, según una reciente publicación se ponen en Bradicardia cuando huyen de un peligro, esto viene a cuento porque este caso caminó de esa manera. Se trata de paciente femenina de 47 años de edad que por causa de un accidente de tránsito, con embestida desde el lado izquierdo, se le produjo trauma en columna cervical, por lo cual se le remitió para una resonancia magnética del cuello que descubrió hernia de disco en vertebra cervical y de paso, presencia de tres nódulos en Tiroides. Curiosamente la paciente niega sintomatologia de patologia tiroidea. Desde Endocrinologia nos remiten el caso para estudio sonográfico de Tiroides, o sea de lo complejo y costoso de una Resonancia Magnética a lo mas simple de una sonografía ( el camino suele ser inverso ).
En la sonografía encontramos un Tiroides francamente patológico, con un aspecto que fue muy bien descrito por la persona que acompañaba al paciente , (parecen cráteres lunares dijo ), esto así porque los dos nódulos solidos, uno de ellos hiper-ecogenico ( vease las dos primeras imagenes ) y el otro hipo-ecogenico, con área central liquida, por necrosis muestran aumento del flujo vascular al Doppler Color en sus áreas periféricas. El primero mide 2.1 X 1.9 X 1.6 cm y el otro 2.8 X 2.3 X 1.9 cm. Junto a esto encontramos cuatro lesiones quísticas de pequeño tamaño.
En el lóbulo izquierdo se visualizan cinco ( 5) imagenes quísticas , la mayor de ellas con 1.2 X 0.9 X 0.8 cm. En estas lesiones el Doppler Color no mostró aumento del flujo vascular.
Para rematar, el marido de la paciente, también asintomático, aprovechando el momento, quiso realizarse una sonografía de Tiroides y le encontramos una imagen quística compleja localizada en lóbulo izquierdo, con múltiples tabiques y una masa solida hiper-ecogenica, redondeada, que crece hacia la luz del quiste.

Endocrinologia refers us to study the case of thyroid ultrasonography, or complex and expensive than an MRI to sonography simplest (usually the opposite way).

We found a Thyroid frankly pathological, with an aspect that was very well described by the person who accompanied the patient, (seem lunar craters said), this thus because both solid nodules, one of them hiper-echogenic one (the two first images see) and the other hypoechogenic, with a liquid central area, by necrosis, shows to increase of the vascular flow in their peripheral areas at the Doppler Color. First measures 2,1 Xs 1,9 1,6 cms and the other 2,8 X 2,3 X 1,9 cms. Next to this, we found four injury cysts of small size. In the left lobe five (5) cystic images, the major of them with 1,2 X 0.9 cms. In these injuries, the Doppler Color did not show an increase in the vascular flow.
To end, the husband of the patient, also asymptomatic, taking advantage of the moment, wanted to be realized sonography of Thyroid and we found a located complex cystic image in his left lobe, with multiple partitions and a hiper-echogenic solid mass, cleared, that grows towards the light of the cyst.
