Enfermedad Dupuytren
Masculino 71 años,diabetico tipo I, con historia laboral de muchos años laborando en el manejo de un montacargas. En el interrogatorio confiesa que tiene curvatura del pene con las erecciones ( Enf de Peyronie ) y ¨durezas¨ en las palmas de ambas manos. El examen físico de estas confirma los de las ¨durezas¨ descritas por paciente en sus manos. El examen de las plantas de los pies no encuentra nada anormal en el derecho. El izquierdo no se examina por estar cubierto de apósitos por fractura en esa extremidad. Tampoco los pabellones auriculares muestran signos de tofos. El examen sonografico de ambas regiones palmares de las manos muestra en la palma de la mano derecha la presencia de dos (2) nódulos sólidos, hipo-ecogenicos, de limites regulares sin sombras o refuerzos ecogenicos posteriores. En la palma de la mano izquierda se visualiza otro nodulo de caracteristicas sonograficas similares. Se concluye con el diagnostico sonografico de Enfermedad de Dupuytren en ambas aponeurosis palmares y el diagnostico clínico de Enfermedad de Peyronie en el pene.Se recalca la importancia de extender el examen físico, el interrogatorio y la exploración sonografica a las áreas de interés, independientemente del pedido inicial del medico referidor ( en este caso la solicitud era de un examen sonografico de abdomen, dicho examen resulto ser normal ).
DUPUYTREN´S DISEASE
71-year-old male, type I diabetic, with many years of work history working in the handling of a forklift. In the interrogation, he confesses that he has a curvature of the penis with erections (Peyronie's disease ) and "skills" in the palms of both hands. The physical examination of these confirms those of the “skills” described by the patient in their hands. The examination of the soles of the feet does not find anything abnormal in the right. The left is not examined because it is covered with dressings due to fractures in that limb. Nor do the ear pavilions show signs of tofos. The sonographic examination of both palmar regions of the hands shows in the palm of the right hand the presence of two (2) solid, hypo-echogenic nodules, of regular boundaries without shadows or subsequent echogenic reinforcements. In the palm of the left hand, another nodule with similar sonographic characteristics is displayed. It concludes with the sonographic diagnosis of Dupuytren's disease in both palmar aponeuroses and the clinical diagnosis of Peyronie's disease in the penis. The importance of extending the physical examination, interrogation, and sonographic examination to the areas of interest, regardless of the initial request of the referring physician (in this case the request was for a sonographic examination of the abdomen, this test turned out to be normal).
