Epididimitis Aguda

 Masculino 27 años edad, con el antecedente de relacion sexual hace 2 semanas. Desde hace 4 dias presenta dolor testicular derecho con presencia de nodulo doloroso en la parte inferior del testículo. Ha tomado por su cuenta antiinflamatorios que le han permitido mejorar levemente el dolor. A la exploración física se palpa masa dolorosa en la parte inferior del testículo derecho. La exploración testicular con transductor sectorial de 10 MHz muestra ambos testiculos con caracteristicas sonograficas normales ( forma, tamaño y ecogenicidad ). El epididimo derecho se aprecia como una masa edematosa, heterogenea, mide aprox: 2.59 X 4.17 cm, a modo comparativo, el epididimo izquierdo mide aprox: 1.17 cm de grosor . La masa muestra aumento del flujo vascular al aplicar el Doppler Color. La Elastografia muestra un score 2 de Ueno con Strain Ratio de 2.88 compatible con lesión benigna. Se concluye con el diagnostico de Epididimitis aguda derecha evidentemente relacionada con enfermedad de transmisión sexual reciente a despecho de que el paciente preguntó de forma ingenua, cual podría ser la causa.

Acute Epididymitis

Male 27 years old, with a history of sexual intercourse 2 weeks ago. For 4 days he has presented with right testicular pain with the presence of a painful nodule in the lower part of the testicle. He has taken anti-inflammatories on his own, which have allowed him to slightly improve the pain. On physical examination, a painful mass was palpated in the lower part of the right testicle. The testicular examination with a 10 MHz sector transducer shows both testicles with normal sonographic characteristics (shape, size, and echogenicity). The right epididymis is seen as an edematous, heterogeneous mass, measuring approx: 2.59 X 4.17 cm, by way of comparison, the left epididymis measuring approx: 1.17 cm thick. The mass shows increased vascular flow when applying Color Doppler. Elastography shows an Ueno score of 2 with a Strain Ratio of 2.88 compatible with a benign lesion. It is concluded with the diagnosis of acute epididymitis on the right, evidently related to a recent sexually transmitted disease, even though the patient asked naively what the cause could be.
