ISTHMOCELE AFTER CESAREAN SECTION: SONOGRAPHIC FINDINGS AND THE VALUE OF TRANSVAGINAL ULTRASOUND
CASE REPORT
SONOGRAPHIC FINDINGS
The uterus is anteverted and measures approximately 6.47 × 3.47 × 3.15 cm.
The endometrium is thin, measuring approximately 1.9 mm.
At the level of the anterior lower uterine segment and corresponding to the cesarean scar, there is an approximately triangular-shaped predominantly fluid-filled defect with irregular internal walls and serpiginous internal contents.
The lesion measures approximately:
2.72 × 1.75 × 1.20 cm
with an estimated volume of approximately:
2.88 mL.
COLOR DOPPLER
Peripheral Doppler flow is identified around the lesion.
This finding may represent vascularization of the myometrium surrounding the defect. Peripheral vascularity alone should not be interpreted as evidence of a vascular complication.
SONOGRAPHIC INTERPRETATION
The location of the defect at the cesarean scar, its triangular morphology and fluid-containing appearance, together with persistent pelvic pain and abnormal bleeding after cesarean delivery, are highly suggestive of a cesarean scar niche or isthmocele.
Isthmocele is also referred to as cesarean scar niche, cesarean scar defect, or cesarean scar disorder.
